Male Facelift Patient 1: This sixty-year-old male patient flew in from Ohio to see me because he was tired of how his jowls and his heavy turkey neck looked. His after photo is only 10 days post-operative and right before he flew back home. He felt that his outer appearance did not match his inner. He felt full of life and energy, but his face made him look tired. A deep plane facelift was performed because for men, if you pull the skin too tight, you will leave them with feminine facial features. My deep plane facelift technique looks natural and is never tight because I lift the underlying subcutaneous fat and facial muscles. To complete the transformation, I performed a neck lift to address his deep jowls and turkey neck. He looks terrific and awake. It is a better, younger looking version of himself. #sixtyisthenewforty
Doctor Jacono specializes in male face lift and neck lift surgery, using advanced less invasive techniques that leave men looking natural and not altered. By using smaller incisions and telescopes, he can minimize visible scarring even for men with balding and short haircuts.
According to the American Academy of Facial Plastic and Reconstructive Surgeons, one of the most common surgeries performed on men are procedures to define and rejuvenate the neck – the Male Mini Neck Lift is on the rise. As men age, the chin and jawline become less defined, with extra fatty tissue depositing itself under the chin. This usually begins in the late thirties and early forties. In the late forties to early fifties, the neck skin begins to sag. As time goes on, a “turkey neck” with large amounts of skin underneath the neck and vertical bands forms. These vertical bands represent the neck muscles called the platysma that loosen with gravity and time.
In male plastic surgery, Doctor Jacono performs the surgery differently than in female plastic surgery in order to maintain a natural appearance that is not feminized. Men are as sensitive about their “turkey neck” as women are, but do not want a face-lift. This procedure has a longer recovery time and requires more surgery. These details are important because men want to look like themselves, simply rejuvenated. There are both non-invasive and minimally invasive surgical treatments that can rejuvenate a man’s neck. Whether a surgical treatment is necessary to tighten the jawline and neck depends upon the degree of loosening, and should be evaluated by Doctor Jacono

The best non-invasive, non-surgical treatment available today is Ultherapy. Ultherapy® is the first and only ultrasound energy-based device for aesthetics cleared by the FDA with a non-invasive “lift” indication for the neck. Ultrasound is sound at frequencies higher than those detected by human hearing (at least 18 kilohertz or 18,000 cycles per second). Ultherapy® addresses the deep skin layers AND the foundational platysma muscle that supports the neck. With ultrasound, we can bypass the upper layers of the skin and deliver the right amount of energy at the right depths to contract and then ultimately lift the platysma. As a result, there is no damage on the surface of the skin, so there is no downtime with this therapy. This triggers the body’s natural response mechanism, which is to rejuvenate tired collagen and supplement it with fresh, new collagen. Collagen is the protein latticework that provides structure to the skin and to the deep supportive connective tissue and muscles below the skin.
For men with more neck drooping that require more tightening than Ultherapy can deliver, or a heavy neck with significant fat deposits, a minimally invasive, isolated neck surgery can be performed.
Excess fat deposits beneath the chin can occur in men of all ages, causing an undesirable contour or the neck, which appears to hang. This area can be sculpted to produce a smoother, more attractive neck and jawline by using liposuction. To remove the fat, a tiny incision is made beneath the chin and behind each ear. Using a small device called a cannula, the fat is suctioned away. As the fat is removed it separates the skin from the underlying muscle. As the skin heals back to the muscles of the neck, it contracts and tightens up. This only works in patients without a lot of extra skin. If there is a lot of extra skin, removing the fat is like deflating a balloon and the skin will hang more.
Male Facelift Before & After Patient 2: This man with hanging neck and jowls wanted a natural appearance without any signs of male plastic surgery or tightness. Dr Jacono specializes in natural appearing facelift and necklift for men with hidden incisions and by lifting the muscle without stretching the skin surface.

Male Facelift Patient 3: This male patient in his late fifties who worked hard his entire life wanted to reclaim his youth. He was concerned with looking tired and wanted to look younger but also look natural. Dr. Jacono performed a combination of facial rejuvenation procedures to accomplish the task. He performed a deep plane facelift, a browlift, upper and lower eyelift, necklift, and rhinoplasty procedure. The result was a refreshed and rejuvenated natural look that has him looking 30 years younger.

Esquire Magazine Dr Jacono was recently featured in the April 2017 issue of Esquire Magazine where he discussed facial plastic surgery for men.
If there is more loosening of the platysma muscle in the neck creating vertical bands, an additional muscle tightening is added. After the liposuction is performed, through the same small incision under the neck, the platysma muscle is tightened with sutures. This is called a platysmaplasty, but this procedure cannot remove large amounts of extra skin.
If you have lots of extra neck skin, it needs to be removed surgically. The least invasive way to accomplish this is with a “direct neck skin excision” where we can make an incision under the chin and remove the hanging neck skin. The immediate effect is that the jawline looks more defined, and it appears that the individual has lost weight because the heaviness under the neck has been removed. A more athletic and energetic appearance is the result. Because of the bearded skin, the incision on the neck heal well and are camouflaged. Doctor Jacono has performed many surgeries like this on men with a busy work schedules, most of whom have only had to take off four or five days from work. A Band-Aid can be placed over the area, so that you can just tell people that you had a cyst removed from your neck. The drawback to this procedure is that the incision is in a more visible spot; the benefit is the shorter recovery.
Male Face Lift Patient 4: This procedure helps treat the jowl and neck. The incision are hidden inside the ear canal so even with short hair there are no visible incisions. The muscle in the neck are repositioned and the skin is not stretched so that it looks natural and not altered like you had a face lift.

Male Face Lift Patient 5: This before and after selfie was sent in from my patient just 2 weeks after his deep plane facelift surgery. He was concerned with his aging appearance but wanted to have natural looking results. To accomplish this, Dr. Andrew Jacono performed his deep plane facelift technique in conjunction with a neck lift with platysmaplasty and a lower eyelid lift (lower blepharoplasty). For men, the results of the facelift procedure should never be too tight because it can feminize the facial appearance and look unnatural. Dr. Jacono lifts the underlying facial muscles (subcutaneous fat) and SMAS, as one unit. This allows the lift to occur from the deep structures, so the skin never appears too tight. He is incredibly pleased with the results.

Male Face Lift Patient 6: This amazing gentleman in his fifties was getting frustrated by the amount of people who were always asking if he was tired. This included a deep plane facelift to lift his cheeks and smooth his jowls. A necklift with platysmaplasty to reduce the excess hanging skin and visible bands on his neck. He is happy that he appears refreshed and he loves his natural results.

For those men who want more hidden incisions, a traditional neck lift is performed, where the incisions are placed behind the ears, and the extra neck skin is lifted and removed in this hidden location. This procedure has a more extended healing time, with bruising and swelling that can last from ten days to two weeks. This lift is different in men because a tighter look needs to be avoided so that the male face is not feminized. This is accomplished by lifting the deeper tissue by going under the muscle, sliding it backwards into its original youthful position, and not by simply lifting and tightening the skin. The muscle is tightened on the sides of the neck and also in the middle of the neck. This technique results in less bruising and swelling since it lifts beneath the muscle and not the skin where all the bruising occurs. Once the deeper tissue is lifted, the excess skin is trimmed. Scars heal better because there is no tension on superficial layers and skin. This lift results in a natural look, and creates definition along the jawline and neck, not tightness.
